With the establishment of individual treatment plans, we use several tools which form part of our integrated model of recovery. Some of these tools will also apply to group activities.

Integral assessments and assignments are conducted throughout the first week of admission. All assessments are prepared to consider critical information when developing their Individual Treatment Plans. Assessments will typically include the following:

Medical and Psychological Assessment

Medical and Psychological Assessment

Our consulting general practitioner conducts a comprehensive physical and psychological evaluation as part of our intake process. This evaluation is meticulously designed to assess not only the individual's current physical well-being but also to delve into the potential harm that may have resulted from the abuse of specific substances or combinations thereof.

Through this thorough assessment, we gain insights into the holistic health of our clients, allowing us to tailor our treatment plans to address their unique needs and circumstances. This initial evaluation serves as a crucial foundation for developing personalized strategies aimed at promoting recovery and mitigating the effects of substance abuse on both the body and mind.

Detoxification and Withdrawal Management

Detoxification and Withdrawal Management

Substance withdrawal management is a critical phase in the journey of recovery, characterized by the safe and supportive management of withdrawal symptoms as individuals abstain from addictive substances.

Recognizing the diverse nature of addiction and the unique needs of each individual, our approach to withdrawal management prioritizes personalization and individualization. our commitment to individualized care extends beyond the acute phase of withdrawal, with ongoing assessment and adjustment of treatment plans as needed. We recognize that recovery is a dynamic and evolving process, and our flexible approach allows us to adapt our interventions to meet the changing needs and goals of each individual as they progress on their journey towards sobriety.

Intake Assessments

Intake Assessments

Our dedicated Social Worker conducts a comprehensive psychosocial assessment as an integral part of our client evaluation process. This assessment extends beyond mere diagnosis, delving into the complex interplay of current life stressors, mental health challenges, relationship dynamics, and overall psychological well-being.

By carefully examining these factors, we gain a holistic understanding of the client's social and emotional landscape, allowing us to tailor our interventions to address their specific needs and foster a supportive environment conducive to lasting recovery.

Assignments

Assignments

Our approach involves reflecting on the profound impact that substance use has exerted across various dimensions of an individual's life, including social interactions, family dynamics, financial stability, and overall health. By meticulously examining these multifaceted domains, we aim to uncover any potential instances of denial surrounding substance use.

This introspective process delves deep into the individual's experiences, shedding light on how substance abuse may have affected their relationships, responsibilities, and personal well-being. Through compassionate inquiry and open dialogue, we create a safe space for individuals to confront any denial they may harbour regarding their substance use. By acknowledging and addressing these underlying issues, we can pave the way for genuine introspection and transformative growth on the path to recovery.

Personality Profiling & Strength Survey

Personality Profiling & Strength Survey

The significance of this assessment lies in its capacity to foster self-reflection and deepen understanding, offering individuals an opportunity for introspective self-reporting on their perceptions of the world and decision-making processes.

By completing this assessment, individuals gain insights into their own thought patterns, beliefs, and behaviours, which are instrumental in informing their journey towards recovery. Moreover, the completion of this assessment enhances the ability to predict relevant information, enabling the multi-disciplinary team to develop tailored treatment strategies that address the unique needs and challenges of each individual. Through this process, individuals can cultivate greater self-awareness, empowering them to make informed choices and embark on a path of meaningful transformation and healing.

Individual Treatment Plan

Individual Treatment Plan

The Individual Treatment Plan (IDP) serves as a comprehensive roadmap that defines and outlines the course of treatment for each individual. Rooted in collaboration between case managers and clients, the IDP articulates clear goals that align with the client's desired outcomes.

These goals are accompanied by measurable, time-sensitive steps designed to facilitate progress towards achieving them. By fostering a shared agreement between case managers and clients, the IDP ensures that treatment efforts are focused, targeted, and tailored to the unique needs and aspirations of each individual, ultimately promoting a collaborative and empowering approach to recovery.

Daily Inventory

Daily Inventory

The daily inventory serves as a vital platform for monitoring both ongoing challenges and progress achieved throughout the program. By systematically documenting experiences and reflections on a daily basis, participants gain valuable insights into their journey of recovery.

This tool allows individuals to track their progress, identify recurring challenges, and celebrate milestones achieved along the way. Moreover, the daily inventory provides a structured framework for self-reflection and accountability, empowering participants to actively engage in their healing process and make informed choices to support their recovery goals.

Family Involvement

Family Involvement

Family assessment and participation are integral to our treatment approach, where we conduct thorough assessments to understand the roles and dynamics within the client's support system. This includes examining nurturing aspects and potential sources of conflict. Engaging family members in treatment planning and decision-making, we create a supportive environment for healing and growth.

Recognizing that every family is unique in its dynamics, history, and cultural background, we emphasize the importance of tailoring treatment approaches to meet the specific needs of each client. By acknowledging and respecting the individuality of each family unit, we can better understand their strengths, challenges, and areas for growth. This understanding forms the basis for developing personalized treatment plans.

Dual-Diagnosis Treatment

Dual-Diagnosis Treatment

Recognizing that individuals with substance use disorders often have co-occurring mental health conditions, such as anxiety, depression, or trauma-related disorders, it becomes imperative to adopt a comprehensive and tailored approach to treatment.

By integrating dual-diagnosis treatment into the individualized plan, we can effectively address the interconnected nature of substance use and mental health issues, providing clients with the specialized care and support they need to achieve lasting recovery.

Through personalized interventions that address both substance use and mental health concerns, we empower individuals to address the root causes of their challenges, cultivate coping strategies, and build a foundation for sustainable wellness and emotional well-being.
